It's probably just creating a new "Version".

It saves new versions of the image as you work, all the time. To go back 
to a previous version or manage versions, choose "Revert to Saved..." 
from the File menu, and you'll get a Time-Machine-esque display showing 
a timeline of all the old versions you have of that file.

If you want to *not* add a version but make a new file, choose 
"Duplicate" after you've opened the image in Preview, before you make 
any changes.

This is a major new feature in Lion that pretty much all the appropriate 
Apple apps implement, and which other apps will adopt with time.
