Registration is now open for the DAIS ITA AFM 10th-12th September. The event is being held at the Oatlands Park Hotel, Oatlands Drive, Weybridge, Surrey, UK. Please complete the attached registration form and return it to Pauline by Wednesday 10th July, early booking is recommended.

Arrival Dates:
The DAIS ITA Annual Fall Meeting begins Tuesday 10th September starting at 9.30am, with Registration and Coffee from 8.30am.  Registration will also be available on Monday evening (6pm - 7.30pm).  Peer reviewers & Leadership team meetings will take place on the Monday 9th September and the 13th September and are by invitation only.
Please note:  Rooms may not be available for check-in until 3pm

Departure Dates:
The AFM is scheduled to end at 5pm on Thursday, 12th September 2019.
Please note: Check-out time is 11am.
A finalised agenda will be sent out before the meeting and distributed to all attendees.
Please complete the attached registration form and return to Pauline (paulinea at uk.ibm.com<mailto:paulinea at uk.ibm.com>) - upon receipt of your completed form, you will receive a link to book directly with the hotel.

Event Meal:
The event dinner will take place on the Wednesday evening at the hotel. All delegates are expected to attend.
The cost of the meal will be £42. There will be an after-dinner speaker.
The cost of this meal is not already included in most rates, (unless you are on the 24-hour delegate rate where it is already included) and will be added as a separate line entry to your overall hotel bill.

Hotel Bookings:
Please book early, we can only guarantee the bedrooms we need up to Wednesday 10th July, so requests for reservations after this date will be on a space available basis.
Any Cancellations or No Shows received after 2nd September will be charged 100% for the first night that you have registered for, to the credit card number provided.
Everyone is responsible for their own expenses, whether staying overnight or just attending the meeting as a day delegate.
Everyone who is a part of the meeting must complete this registration form and check-in at the lobby — this includes speakers and attendees.  This applies even if you are only attending or speaking for a portion of the day.
Full payment will be taken from your nominated payment method upon checkout from the Hotel.

Oatlands Park Hotel is 2 miles from the M25 (Junction 11) and also just minutes from the A3 and M3 motorway.

Closest Train station
The nearest railway stations are Weybridge (1 mile) and Walton-on-Thames (1.8 miles).  South West Trains take 30 mins to Weybridge and 26 mins to Walton-on-Thames from London Waterloo.
